From a physician’s perspective, the patient encounter form is the most important part of an electronic medical record system. It is the same for psychiatry practices; encounter form is the place where a psychiatrist would usually spend most of his/her time and therefore its design must facilitate fast and effective documentation. The encounter form on our EMR system displays all patients’ visit related information in a structured manner on one single longitudinal form. This design feature has many advantages to it. At the least, it eliminates the need to back out to various sections of the EMR program to or enter or retrieve information.
