The system would automatically detect any Axis... prefixes on the encounter form and, if found, would format the Assessment/Diagnosis part of the encounter/consult note in multi-axial format. Obviously not every ICD code on the favorite list would have an association with Axis and therefore there would be many codes without any Axis prefix. If any ICD code is found without the Axis prefix, it would still be included on the resulting note. All such codes would be grouped together neatly at the bottom of the Assessment/Diagnosis section.
Finally, please note that it is not necessary to generate notes in multi-axial format as it just adds a layer of complexity for the user. Many psychiatrists still prefer the sequential note format that is standard for all medical specialties. The generation of multi-axial notes is just one of the many features available in UniCharts electronic medical records system and it may be ignored by the physician if not needed.
