If you prefer to have the assessment part on the generated encounter or consult notes shown in multi-axial format, you just need to add the Axis number before its ICD description when building the ICD favorite list, as shown in the above screenshot. The prefixes would take the form of [Axis I], [Axis II], [Axis III] respectively. Please note that there would be no prefixes [Axis IV] and [Axis V] as these will go into separate blocks on the encounter form. Also, the prefixes should be added carefully because, as far as the computing is concerned, [Axis I] is not the same thing as [AxisI], [Axis 1], [Axis l] or [AXIS I]. The program logic has been implemented to process the terms [Axis I], [Axis II] and [Axis III] in the ICD prefixes and Axis IV and Axis V as the block names.
