The percentage of patients 18 years of age and older who were diagnosed with a new episode of major depression, treated with antidepressant medication, and who remained on an antidepressant medication treatment.

Description and Implementation within the EMR software

There is 1 denominator having 2 numerators in this measure and this translates to 2 sets in total.

Denominator: All patients 18 years or older with at least 1 encounter during the meaningful-use reporting period and having one of the following ICD-10 codes selected in ASSESSMENT/DIAGNOSIS section on the ENCOUNTER form:
F32.0, F32.1, F32.2, F32.3, F32.4, F32.9, F33.0, F33.1, F33.2, F33.3, F33.40, F33.41, F33.9, F34.1, F43.21

Numerator 1: Patients in the above mentioned list to whom a follow up on antidepressant medication treatment was provided and the cumulative medication duration is more than 83 days and less than 115 days. UniCharts looks for the custom code QM105B: [Quality Measure] Followup: Antidepressant medications prescribed. CUMULATIVE medication duration is more than 83 days and less than 115 days within the PROCEDURES/SERVICES section on the ENCOUNTER form as the required documentation.

Numerator 2: Patients in the above mentioned list to whom a follow up on antidepressant medication treatment was provided and the cumulative medication duration is more than 179 days and less than 232 days. UniCharts looks for the custom code QM105C: [Quality Measure] Followup: Antidepressant medications prescribed. CUMULATIVE medication duration is more than 179 days and less than 232 days within the PROCEDURES/SERVICES section on the ENCOUNTER form as the required documentation.

Note that in case of a newly diagnosed depression episode in which you have prescribed antidepressant medications, you should select the custom code QM105A: [Quality Measure] New episode of major depression. Acute phase antidepressant medications prescribed (G8126) within the PROCEDURES/SERVICES section on the ENCOUNTER form. Similarly, you would select the custom code QM105X: [Exclusion] Followup: Antidepressant medication prescribed. CUMULATIVE duration does not apply as last prescription was less than 91 days ago if it is a followup visit but cumulative medication duration does not apply. This would make the episode eligible for denominator exclusion
