Heart Failure (HF): Beta-Blocker Therapy for Left Ventricular Systolic Dysfunction (LVSD)

Percentage of patients aged 18 years and older with a diagnosis of heart failure who also have LVSD (LVEF < 40%) and who were prescribed beta-blocker therapy.

Description and Implementation within the EMR software

Denominator: All patients 18 years or older with at least 2 encounters during the meaningful-use reporting period and having one of the following ICD-10 codes selected in ASSESSMENT/DIAGNOSIS section on the ENCOUNTER form:
I11.0, I13.0, I13.2, I50.1, I50.20, I50.21, I50.22, I50.23, I50.30, I50.31, I50.32, I50.33, I50.40, I50.41, I50.42, I50.43, I50.9
AND who also have LVSD (LVEF < 40%)

Numerator: Patients in the above mentioned list to whom beta-blocker therapy such as Atenolol, Betapace, Coreg, etc was prescribed. UniCharts looks for the custom code QM083A: [Quality Measure] HF patient with LVSD, beta-blocker therapy not prescribed or QM083B: [Quality Measure] HF patient with LVSD, beta-blocker therapy prescribed (G8450) or QM083C: [Quality Measure] HF patient with NO LVSD (LVEF within normal range) within the PROCEDURES/SERVICES section on the ENCOUNTER form as the required documentation.

Exceptions: UniCharts looks for the custom code QM083E: [Exception] Action specified in CMS144v2 not performed due to medical, patient, or system reasons within the PROCEDURES/SERVICES section on the ENCOUNTER form as the required documentation.
