Functional Status Assessment for complex chronic conditions

Percentage of patients aged 65 years and older with a diagnosis of heart failure who have been seen for at least 2 office visits, with functional status assessment results present in the EMR.

Description and Implementation within the EMR software

Denominator: All patients 65 years or older with at least 2 encounters during the meaningful-use reporting period and having one of the following ICD-10 codes selected in ASSESSMENT/DIAGNOSIS section on the ENCOUNTER form:
I11.0, I13.0, I13.2, I50.1, I50.20, I50.21, I50.22, I50.23, I50.30, I50.31, I50.32, I50.33, I50.40, I50.41, I50.42, I50.43, I50.9

Numerator: Patients in the above mentioned list who has functional status assessment results such as VR-12, VR-36, MLHF-Q, KCCQ, PROMIS-10 Global Health or PROMIS-29 present in the EMR. UniCharts looks for the custom code QM992A: [Quality Measure] Functional status assessment completed and results present within the PROCEDURES/SERVICES section on the ENCOUNTER form as the required documentation. If the functional status assessment was not completed due to severe dementia, select QM992X: [Exclusion] Functional status assessment NOT completed due to an active diagnosis of severe Dementia instead. Similarly, if the functional status assessment was not done due to cancer, select QM992Y: [Exclusion] Functional status assessment NOT completed due to an active diagnosis of Cancer instead.
