Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention

Percentage of patients aged 18 years and older who have been seen for at least 2 office visits who were screened for tobacco use one or more times within 24 months AND who received cessation counseling intervention if identified as a tobacco user.

Description and Implementation within the EMR software

Denominator: All patients 18 years or older with at least 2 encounters within the reporting year.

Numerator: Patients in the above mentioned list who are identified as non-smokers AND those who are identified as smokers but were provided cessation counseling or cessation pharmacotherapy. UniCharts looks at the SMOKING STATUS field within HEALTH MAINTENANCE section on the HISTORY form to determine whether the patient was identified as non-smoker or a smoker. To determine whether a patient in the above list received counseling or a prescription for smoking cessation agent, UniCharts looks for the custom code QM028A: [Quality Measure] Patient screened for tobacco use and received cessation counseling (4004F) within the PROCEDURES/SERVICES section on the ENCOUNTER form as the required documentation.

Exceptions: UniCharts looks for the custom code QM028E: [Exception] Action specified in CMS138v2 not performed due to medical, patient, or system reasons within the PROCEDURES/SERVICES section on the ENCOUNTER form as the required documentation. In addition, the system automatically grants exception to patients with SNOMED code 162607003 (Limited Life Expectancy) in the ASSESSMENT/DIAGNOSIS section on the ENCOUNTER form.
