Controlling High Blood Pressure

The percentage of patients 18-85 years of age who had a diagnosis of hypertension and whose BP was adequately controlled during the measurement year.

Description and Implementation within the EMR software

Denominator: All patients 18-85 years of age with at least 1 encounter during the meaningful-use reporting period and having one of the following ICD-10 codes selected in ASSESSMENT/DIAGNOSIS section on the ENCOUNTER form:
I10, I11.0, I11.9, I12.0, I12.9, I13.0, I13.10, I13.11, I13.2

Numerator: Patients in the above mentioned list whose last blood pressure reading during the meaningful-use reporting period was less than 140/90 mmHg. UniCharts search the VITAL SIGNS section on the ENCOUNTER form to extract the last blood pressure reading of the patient.

Exceptions: None

Note: The system will automatically exclude the patients with ICD codes N18.5 (Chronic kidney disease, stage 5), N18.6 (End stage renal disease), Z48.22 (Encounter for aftercare following kidney transplant), Z94.0 (Kidney transplant status), Z99.2 (Dependence on renal dialysis), O00.1 (Tubal pregnancy), Z33.1 (Pregnant state, incidental) and O62.3 (Precipitate labor) from the denominator. The system will also look for CPT code 90920 (Dialysis Services) within the PROCEDURES/SERVICES section on the ENCOUNTER form and exclude the patient if found.
