Heart Failure (HF): Angiotensin-Converting Enzyme (ACE) Inhibitor or Angiotensin Receptor Blocker (ARB) Therapy for Left Ventricular Systolic Dysfunction (LVSD)
Percentage of male and female patients aged 18 years and older with a diagnosis of heart failure and LVSD (LVEF <40%) who were prescribed ACE inhibitor or ARB therapy.

Denominator: All male and female patients 18 years or older with at least 1 encounter during the reporting period having one of the following diagnosis codes selected in ASSESSMENT/DIAGNOSIS section:
402.01, 402.11, 402.91, 404.01, 404.03, 404.11, 404.13, 404.91, 404.93, 428.0, 428.1, 428.20, 428.21, 428.22, 428.23, 428.30, 428.31, 428.32, 428.33, 428.40, 428.41, 428.42, 428.43, 428.9
Numerator: Patients in the above list to whom ACE inhibitor or ARB therapy such as Accupril, Diovan, Micardis, etc was prescribed. UniCharts EMR looks for the custom code QM081A: [Quality Measure] HF and LVSD patient, ACE inhibitor or ARB therapy prescribed (4009F) within the DIAGNOSTICS/PROCEDURES section on the ENCOUNTER form as the required documentation.
Exclusions: UniCharts EMR looks for the custom code QM081E: [Exclusion] Action specified in NFQ 0081 not performed due to medical, patient, or system reasons within the DIAGNOSTICS/PROCEDURES section on the ENCOUNTER form as the required documentation.
